Float Tube Adventures Trip Waiver

PLEASE READ CAREFULLY - This is a waiver and release of liability

I, the undersigned, hereby acknowledge: 

   1. That I fully understand and accept that there are certain hazards 
      and elements of danger inherent in many if not all activities
      which are beyond the control of the trip coordinators, guides, agents, 
      and employees of Float Tube Adventures. 
   2. I realize that my participation in these activities could result in 
      personal illness or injury due to accidents, the forces of nature,
      and other causes not foreseeable. Such illness and injury may include 
      disease, strains, sprains, fractures, dislocations,
      paralysis, and/or death. Possible injuires may result in grave 
      and/or permanent disability. 
   3. I also realize that my participation in these various activities 
      may result in the loss or damage to personal property. 
   4. Further, I Understand that no one but myself is responsible for judging 
      my qualifications or for my safety when I choose to
      participate in this activity. 

I hereby waive for myself and anyone else claiming
through me, my right to sue or recover damages from Float Tube Adventures, 
its members, officers, directors, trip coordinators, guides, agents, 
and employees for any illness or injury to my person, loss of life, 
and any damage or loss of personal property which may arise out of my 
participation in the Float Tube Adventure activities. 

The waiver applies to any negligent act or omission and to any intentional 
act intended to promote my safety or well-being. I further state that I am 
legally competent to sign this waiver and release of liability; and that I 
understand that the terms herein are contractual and mere recital. 
I have read this waiver before signing it, and I have signed it voluntarily.

Parental or Guardian's Release of All Claims and Covenant Not to Sue 
The undersigned being the parent(s), guardian, or person having the 
care and custody of ________________________, do hereby
consent that s/he may participate in the activites provided by 
Float Tube Adventures and in consideration of Float Tube Adventures, 
its officers, agents and employees permitting s/he to so participate, 
do hereby covenant and agree not to sue Float Tube Adventures or 
its officers, agents, or employees for any claim which may arise out of the 
Float Tube Adventures activities. 

I hereby certify that I have full knowledge of the nature and extent of 
the equipment / conditions that Float Tube Adventures provides
and that my child is voluntarily assuming the risks. I understand that I 
will by solely responsible for any loss or damage, including death, 
my child sustains while using Float Tube Adventures Guide Service
and that by this agreement I am relieving Float Tube Adventures of any a
nd all liability for such loss, damage, or death. 

I further certify that my child is in good health and that s/he has no 
physical limitations which would preclude the activities provided 
by Float Tube Adventures. 

BY SIGNING THIS DOCUMENT, IT IS OUR INTENTION TO EXEMPT AND RELIEVE 
FLOAT TUBE ADVENTURES, ITS OFFICERS, AGENTS, OR EMPLOYEES FROM LIABILITY 
FOR PERSONAL INJURY, PROPERTY DAMAGE, OR WRONGFUL DEATH CAUSED BY
NEGLIGENCE..
